Job Description

80 Hours per 2 weeks pay period - Salaried Position Located in Seymour, WI 15-20 minutes from Green Bay or Appleton Good Shepherd Home has been a 5-star CMS facility for years! The primary purpose of this position is to plan, organize, develop, and direct the operation of the Social Services Department in accordance with current federal, state and local standards, guidelines and regulations, our established polices and procedures, and as may be directed by the nursing home administrator/Executive Director to assure that the medically related emotional and social needs of Good Shepherd clients, residents are met/maintained on an individual basis. A customer service orientation toward both internal and external customers is required. The position is accountable for all facets of the resident admission and discharge processes.

EDUCATION

Preferred Social Work Degree

EXPERIENCE

Must have a minimum of one (1) year of work experience in long-term care setting.

SOCIAL WORK FUNCTIONS

Participate in the completion of the Minimum Data Set with the interdisciplinary team. Participate and complete an appropriate plan of care for each resident using resident/family input as appropriate. Review and revise plans of care quarterly and as necessary. Ensure admissions, transfers and discharges are completed per regulatory and Good Shepherd guidelines. Facilitate the investigation and complaint processes. Offer input during resident care planning sessions. Complete all required social service documentation in a professional and timely fashion. Documentation will include social histories, admission assessments, care plans, quarterly notes, MDS interviews, Level 1 screens, discharge planning, Mini Mental State Examinations, depression scales and other social service notes as needed.
