Job Description

Role Summary: Our client is seeking a compassionate and skilled Social Worker to join their interdisciplinary team dedicated to providing comprehensive care for elderly patients. As an integral member of the care team, you will deliver essential social work services, including assessments, care planning, counseling, and case management. By collaborating with patients, their families, and other care team members, you will help patients achieve their full potential and remain safely in their communities. Population: Geriatrics Essential Duties and Responsibilities: Conduct thorough psychosocial assessments to identify the needs, goals, and preferences of patients. Actively participate in IDT meetings to develop and implement patient care plans. Deliver and document social work interventions, including resource coordination, care transitions, counseling (individual and group), and case management. Collaborate with primary care physicians and care teams to ensure smooth transitions between care settings (e.g., hospitals, skilled nursing facilities, and home). Facilitate care conference meetings to promote effective communication among patients, families, and the care team. Provide discharge planning for patients transitioning out of the program. Other duties as assigned. Qualifications: Master’s Degree in Social Work (MSW) required. At least one (1) year of experience working with elderly or frail populations. Experience coordinating and facilitating care conference meetings. Experience assisting individuals with behavioral health and substance abuse issues preferred. Additional Information: Full time direct hire employment with additional compensation: Bonus + Equity. Pay will be adjusted based on experience and qualifications.
